Coach B requires all campers to upload a recent (within 18 months of the start of camp) Physical and Immunization record signed by a physician.

Also, for campers who will need medication at camp, a parent/guardian needs to download the Authorization to Administer Medication (linked below), complete and sign, and upload it here.
